Indianapolis – Man arrested. DUI charged after fatal crash involving church van | indystar.com

An Indianapolis man has been arrested on suspicion of leaving the scene of an accident that killed the driver of a church van and injured several others on board Sunday night on the Northeast side.

Brandon Collins, 26, has been preliminarily charged with drunken driving and leaving the scene of an accident, the Indianapolis Metropolitan Police Department said. The accident occurred just before 8 p.m. at 46th Street and Emerson Avenue.

The driver of the church van, 51-year-old Edward Sadler, was pronounced dead at the scene.

A Spencer woman died Sunday night from injuries received in a traffic accident that afternoon west of Bloomington, and the man driving the mini van that collided with her motorcycle has been charged with driving while intoxicated resulting in death.

Tammy M. Hale, 51, was eastbound in the 9000 block of West Ind. 48 on a 2007 Yamaha motorcycle when a 1997 Chevrolet Venture van driven by 49-year-old Larry S. Uland of Solsberry, headed west, crossed the center line and hit the motorcycle head-on, according to a report from the Monroe County Sheriff’s Office.

Howard County authorities say alcohol may have involved in a fatal crash on U.S. 31 which killed one man.

Just before 1 a.m. Thursday, Howard County dispatchers were notified of a possible intoxicated driver heading northbound on U.S. 31. The witness stated the vehicle was traveling in excess of 70 mph and swerving in and out of traffic.

At approximately 1:05 a.m., a resident living near the U.S. 31 bypass in Tipton County called 911 saying she\’d heard an explosion and observed an object burning on the roadway. Deputies responded, and upon their arrival located a truck engulfed in flames which matched the previous caller\’s description.

Deputies said evidence at the scene indicated the vehicle had been traveling northbound in the southbound lane at a high rate of speed before striking a crane parked on the roadway. The impact of the crash caused the crane to be dislodged several feet before coming to rest on the bridge railing.

Once the flames were extinguished, deputies located a single deceased occupant in the driver\’s seat.

Toxicology tests revealed a Union City man killed in a Nov. 6 traffic accident was intoxicated at the time of the crash.

Jason Gibson, 36, Union City, was pronounced dead at the scene of the accident, on U.S. 27 south of Bryant.

Authorities said Gibson’s northbound pickup truck crossed the center line and collided head-on with a southbound sport-utilityvehicle driven by Joshua Hall, 25, of Nashville, Indiana.

Hall and his passenger, 26-year-old Christina Bishop – also listed as a Nashville resident, although she and Hall own businesses in Portland – were severely injured in the accident.

Jay County authorities said this week that Gibson’s blood-alcohol content at the time of the accident was 0.08 percent. In Indiana, motorists with a BAC of 0.08 percent or higher are considered intoxicated.

The toxicology report also showed the Union City man had ingested methamphetamine and marijuana.

Authorities have identified the victim in a fatal crash Wednesday afternoon as Brian Poe, 38, Trafalgar.

Poe was a passenger in a 1998 Dodge pickup driven by Devin Fleener, 18, Johnson County Sheriff Doug Cox said in a news release.

The crash happened around 2:07 p.m. on County Road 300 South, near Fairhaven Christian Church. Police found the truck upside down.

Poe was pronounced dead at the scene. He was thrown from the truck during the crash, police said.

The survivor first told police that Poe was driving, Cox said, but later admitted to being the driver.

Blood tests on Fleener are pending. A portable breath test showed he had a blood-alcohol level of 0.039, according to a police report. That is less than the 0.08 at which a driver is considered ­impaired in Indiana, but police arrested Fleener on a preliminary charge of being a minor in possession of alcohol, Cox said.

Alcoholic drink containers were found near the truck, police said.

A 17-year-old girl was killed early Saturday when a drunk driver who police said was fleeing the scene of a shooting ran a red light and struck her car. Haley M. Nellum, a sophomore at Bishop Dwenger High School, died at the scene of the crash at Stellhorn and Hobson roads. The driver of the car that ran the red light, Jeremy D. Washington, 35, is facing preliminary charges of operating while intoxicated and causing death, aggravated battery and criminal recklessness causing serious bodily injury. Police said that just after midnight they were called to the Corner Pocket Pub at 3215 St. Joe Center Road on reports someone had fired a gun. Witnesses said they heard gunshots and saw two cars leave the parking lot southbound at high speed. The crash at Hobson and Stellhorn occurred about three minutes later. Hobson Road becomes St. Joe Road north of Stellhorn. A few minutes after the crash, a car carrying a shooting victim showed up at the emergency room of a local hospital. The car he was riding in also had bullet holes in it, police said. The man told police he had been shot in the parking lot of the Corner Pocket Pub. Police determined all three events were related. They said occupants of the car that ultimately was involved in the crash got into an argument with some people in another car in the parking lot of the bar and opened fire. Both cars then fled at high speed. Witnesses to the crash said the vehicle that was carrying the alleged shooter ran a red light at Stellhorn and Hobson and struck Nellum’s car, which was traveling east and was not involved in the shooting. Police said that at the scene of the crash, Washington was able to get out of the car but that a passenger was trapped and had to be removed by the fire department. Washington was taken to a local hospital in fair condition and his passenger was taken to a hospital in serious condition. Meanwhile, the shooting victim was transferred to another hospital in serious condition. Police said they are interviewing witnesses and that Washington is in custody. Nellum, who had played junior varsity and some varsity basketball and volleyball at Dwenger, was apparently just on her way home from visiting a friend when the crash happened. Her mother, Laura Nellum, said her daughter had gotten her driver’s license only eight days before. The crash happened about a half-mile from where Nellum lived. She said her daughter loved sports and participated in them at every opportunity. She helped with elementary students and had helped her sister with the Special Olympics recently. Sports weren’t part of her long-term goal, though. She had recently expressed an interest in becoming a nutritionist. Cleveland Inge, girls basketball coach at Dwenger, described Nellum as “a really good kid, an outstanding kid” who helped out with the summer league middle-school teams. “She was a very well liked player,” Inge said. “Everybody loved her. She was always smiling all the time and had a positive attitude always,” he added. An off-duty police officer was arrested on OWI charges after a fatal accident Sunday afternoon.

The accident occurred around 12:20 p.m. in the 7500 block of State Road 32 West.

Edgewood Police Officer James Foutch was driving a GMC Yukon WB on SR 32 when he rear-ended a Buick Century. The force of the impact pushed the Buick Century off the north side of the roadway and into a utility pole.

Foutch was uninjured; his passenger was transported to St. Vincent Hospital Anderson with complaint of pain.

Rebecca Marie Sperry, the driver of the Buick, was nine months pregnant, and she was transported by Stat Flight Medical Helicopter with severe injuries to St. Vincent Hospital Indianapolis. Her husband Jesse Sperry –the passenger in the Buick–was pronounced dead at the scene.

Later Monday, Rebecca Sperry gave birth to a baby girl, according to family members on a YouCaring page.

Foutch was transported to St. Vincent Hospital Anderson for a blood draw, and he was later arrested on charges of operating while intoxicated causing death.

He posted the required $20,000 bond.

Police are still investigating the accident.

Avon police are investigating an accident late Saturday night that killed a Cicero woman.

Authorities say that after visiting friends in Avon, Brandon Broady and his wife Jessica got into an argument about where they were going next. Investigators believe Jessica opened the truck door and attempted to jump from the moving vehicle. As she fell to the ground, police say she was run over by the rear wheels of the truck.

Jessica was transported to a hospital, but died from her injuries. An autopsy found multiple skull fractures to be the cause of death.

Brandon Broady failed a field sobriety test, according to investigators. A blood draw was performed and the results are not yet available. As a result of the failed field sobriety test, Brandon was preliminarily charged with driving while intoxicated resulting in death.

Brandon was released on a $2500 cash bond. He will appear in a Hendricks County court once formal charges are filed.

At this time, police believe that the death appears to be accidental. They do not think that Brandon pushed Jessica, nor do they think Jessica was attempting to commit suicide.

A woman is dead after an early Sunday morning car crash in Osceola.

The crash happened in a cul-de-sac near Chestnut, just south of Adams.

Five people got into the victim’s minivan after a gathering at a nearby house. The 24-year-old woman sat in the front passenger seat, along with another person.

Investigators believe the victim had her head out of the window because she was getting sick. Her head collided with an object outside the car, according to Capt. Tim Spencer from the St. Joseph County F.A.C.T team. The woman suffered fatal injuries.

A passenger in the car called police for help but the driver attempted to flee the scene.

A Mishawaka police officer apprehended the driver shortly after.

A woman is now under arrest after a car crash that killed one man.

Vanderburgh County Sheriff’s Office arrested Maria Stevens on charges of Operating a Motor Vehicle while Intoxicated causing Death.

The arrest comes after the crash on June 15 at New Harmony Road just west of Koressel Road. Authorities originally thought Ronald Boback, who died in the crash, was the driver of the vehicle.

But, an investigation revealed that  Stevens was driving the vehicle when crashed, ejecting Boback from the vehicle.

Investigators found that Stevens had a 0.12 blood alcohol content. Stevens has been taken into custody at the Vanderburgh County Confinement Center and remains there until her initial court appearance.

Christopher Payne is facing a boating while intoxicated resulting in death charge after 19 year old Garret Ward died after being caught in a boat propeller. Investigators say Ward and some friends had stopped at French Islands and around 6pm Sunday they headed back out on the Ohio River. Officials say Payne started to drive away from the island and Ward got caught in the propeller. Ward’s autopsy is set for Monday afternoon, Payne remains in the Spencer County Jail.

Alcohol is suspected in a Friday night crash that left a 66-year-old Northwest Indiana woman dead and her husband critically injured, the Post-Tribune is reporting.

Theresa Johnson of Valparaiso died early Saturday morning at Porter Regional Hospital where she was being treated for severe internal injuries suffered in the crash, Sgt. Larry LaFlower, public information officers with the Porter County Sheriff’s Department said in a release Saturday.

Porter County Coroner Chuck Harris said Johnson was pronounced dead at 12:44 a.m. Her husband, Dwayne Johnson, 67, was taken to St. Mary’s Hospital and Medical Center in Hobart and is in critical but stable condition, Harris said.

The crash occurred at about 9:45 p.m. Friday when the car driven by Dwayne Johnson was headed northbound on 250 West in Center Township, Ind., when police said he disregarded the red light at the U.S. 30 intersection and crashed into car carrying a family that was heading eastbound through the intersection.

The 41-year-old Warsaw man who was driving the eastbound vehicle was not seriously injured. His 34-year-old wife suffered minor injuries and was transported to St. Mary’s Medical Center in Hobart.

Two children, 5 and 1, were in child safety seats in the back seat. The children suffered minor injuries, LaFlower said.Both drivers were tested at the scene for alcohol consumption. The driver of the eastbound vehicle tested negative. Johnson came back with alcohol in his system, police said. A search warrant was issued for a blood draw at the hospital.

Charges are pending because Johnson still in the hospital, LaFlower said.

Indiana State Police are investigating a two-vehicle crash that left one person dead and three injured near Madison just before 5 p.m. Sunday.

Police said a Toyota pick up driven by 55-year-old James Bagshaw was traveling west on State Road 62 when it crossed the center line into oncoming traffic.

Bagshaw’s vehicle crossed the eastbound lane, police said, went off the south side of the road and struck a utility pole causing it to spin and hit a Ford Fusion.  The vehicle then struck a guardrail, causing a piece of the guardrail to enter the truck. The piece of rail struck Bagshaw, ejecting him out of the rear window.

Bagshaw was pronounced dead at the scene. A passenger in the truck with Bagshaw was treated at the scene for minor injuries.

The elderly driver and the passenger of the Ford Fusion were taken to the hospital and treated for minor injuries.Police said Bagshaw was traveling from the Madison Regatta. They said they alcohol and marijuana may be factors in the crash. Toxicology results are pending.

A 25-year-old Amboy man was killed Sunday morning in a single-vehicle crash after police say an intoxicated driver crashed a truck.

The Converse Police Department responded to the accident at around 12:44 a.m. near the intersection of 950 East and 1200 South in Miami County.

Police said they found a 2000 Dodge Ram driven by 26-year-old Bryan Zirkle, Converse, had traveled off the road and overturned.

Kyle Page, of Amboy, a passenger in the truck, died from injuries suffered in the accident.

Zirkle was taken to Dukes Memorial Hospital, where he was treated and released for his injuries. Police then arrested him for operating while intoxicated causing death and incarcerated at the Miami County Jail.

Police say the crash remains under investigation.

Agencies assisting at the scene included the Indiana State Police, Miami County Sheriff’s Department, Amboy Fire Department, Converse Ambulance and Dukes Memorial Hospital.

Vigo Co., Ind.: Drunk driver arrested after fatal crash in Vigo County | WISH-TV

Officials said alcohol and drugs were involved in a fatal rollover crash in northern Vigo County early Saturday morning.

Just after 1 a.m., Vigo County deputies were called to the area of 42nd Avenue and Wire Street for an accident with entrapment. They arrived to find a black Humvee upside down in a field.

According to witnesses, Kyle Pendergast, of Terre Haute, was driving with six other people in the vehicle in a hayfield when it rolled.

Alyssa Hogue, 19, also of Terre Haute, was thrown from the vehicle and trapped when the vehicle rolled. She was pronounced dead at the scene.

A blood test, which is required by Indiana Code, determined Pendergast had a blood alcohol content BAC of .108 percent. Police said Pendergast also tested positive for marijuana.

He was arrested and taken to the Vigo County Jail for operating a vehicle while intoxicated, resulting in death.

A LaPorte man was allegedly drunk when he was driving a pick up truck that struck and killed a man on a moped in Michigan City Thursday night, police said.

Jordan Dant, 29, of Michigan City, was pronounced dead at the scene of the crash, LaPorte County Coroner John Sullivan said Friday morning. The cause of death was blunt force trauma suffered in a motor vehicle accident, Sullivan said.

Ryan Garrett, 40, of LaPorte was charged with operating while intoxicated causing death, a level 4 felony, after the crash, Michigan City Police Department officials said.

Police were called at 9:21 p.m. to Michigan Boulevard and Warnke Road for a report of a man injured in an auto accident. When officers arrived, they found Dant lying in the eastbound lane of Michigan Boulevard.

Dant died before LaPorte County EMS units arrived, police said.Investigators said Dant was on his moped trying to exit the driveway of Sand Trap Lounge, 3015 E. Michigan Blvd. when he pulled into the path of Garrett’s 2004 Chevy 3500 pickup truck. Garrett was not injured in the crash.

Garrett was transported to Franciscan St. Anthony Health hospital in Michigan City for drug and alcohol testing, which revealed he was legally intoxicated at the time of the crash, police said.

Two people were killed and a third was taken to an Indianapolis hospital after a collision near Geist Reservoir Saturday night.

Around 11 p.m. Saturday, emergency crews were dispatched to a two-car collision near the intersection of Fall Creek Parkway and Sand Key Lane, just south of 96th Street.

IMPD spokesman Rafael Diaz said witnesses told police a Cadillac crossed the center line, striking a Ford Explorer with three people inside.

The driver and a passenger sitting in the rear of the Ford were declared dead at the scene, Diaz said. The third passenger was taken to an Indianapolis hospital.

The deceased driver and passenger were later identified as David Foster, 50, and Whitney Foster, 28, of Fortville, Indiana.

The driver of the Cadillac — Paige Byers, 22, of Noblesville — was removed from her vehicle by by-standers, and she was taken to the hospital with minor injuries, police said.

Byers was arrested Sunday on preliminary charges of two counts of Operating a Vehicle while Intoxicated causing Death and one count of Operating a Vehicle while Intoxicated causing Serious Bodily Injury.

The case will be forwarded to the prosecutor’s office for a decision on whether or not formal charges will be filed.

A Millersburg man was intoxicated when he fatally hit an 80-year-old bicyclist in LaGrange County, police say.

Officers from the LaGrange County Sheriff’s Department were called to the scene near 6800 W. C.R. 200 S. just after 8 p.m. Monday, Sept. 22, and found 80-year-old Leland Lambright of Emma unresponsive. He was pronounced dead at the scene, according to a news release from the sheriff’s department.

Investigators arrested 22-year-old Brian Schrock of Millersburg in connection to the incident. He was driving a 2002 GMC Envoy while using a cell phone and under the influence of alcohol when he hit Lambright, according to the news release.

He faces preliminary charges of operating a vehicle while intoxicated causing death and reckless homicide.

The LaPorte County Sheriff’s Office and the LaPorte County Fatal Alcohol Crash Team FACT are investigating a single vehicle fatality crash that occurred in rural LaPorte County on Wednesday, September 24.

At approximately 9:50 pm CST deputies from the LaPorte County Sheriff’s Office were summoned to the 900 North block of County Road 640 West in reference to a single vehicle crash with injuries.

Upon arrival deputies located a 2006 Nissan four door passenger car that had left the roadway and had impacted a tree.

The vehicle was occupied by the driver and a single male passenger and the passenger was unconscious and subsequently transported to St. Anthony Memorial Hospital in Michigan City where he was pronounced dead.

He has been identified as 47-year-old Lanny Franks of rural LaPorte.

The driver suffered minor injuries and deputies established probable cause at the scene to believe that he was impaired. It was soon determined that he had a blood alcohol content that was in excess of Indiana’s limit of .08%.

Because a death had resulted from the crash and because alcohol was a contributing factor, the LaPorte County FACT Team was summoned to the scene and the crash remains under investigation.

The driver has been identified as 27-year-old Daniel J. Grigalanz of Michigan City. He has been charged with three misdemeanor counts of Operating While Intoxicated as well as Operating While Intoxicated – Causing Death, a Level 4 Felony and Operating While Intoxicated – Previous Conviction, a Level 6 Felony.

He is currently being held in the LaPorte County Jail and he is scheduled to appear in LaPorte County Circuit on September 26. His bond has been set in the amount of $2,005.

The LaPorte County Sheriff’s Office was assisted at the scene by Coolspring Fire Department and LaPorte County Emergency Medical Service.

Indiana State Police say an alcohol-related head-on crash in southern Indiana has killed a Kansas man and injured six other people.

Sgt. Noel Houze says a car driven by 23-year-old Jordan Griffin of Versailles went left of center about 4 p.m. Saturday on U.S. 421 north of Madison and struck a pickup truck driven by 37-year-old Charles McRoberts Jr. of Versailles.

Griffin’s passenger, John Thomas Walker, of Arkansas City, Kansas, died at the scene. His name hasn’t been released.

Both drivers and four children — three 11-year-olds and a 2-year-old — riding in the truck all were hospitalized with injuries.

Houze says Griffin had a blood-alcohol content of 0.41 and will face charges.

Griffin’s father said Sunday that his son remained hospitalized and didn’t have an attorney.
